You must call VDC Electronics Technical Support to receive an RMA number prior to filling out this form or sending any unit back. Purchase date must be within 5 years of purchase. Return forms received without an RMA Number will not processed. Please pack your BatteryMINDer carefully and include a copy of your original reciept. If your original reciept is not available, our technicians will determine the age of your unit when you call.

If instructed to return your BatteryMINDer, please make sure that is in clean condition.
